Multi-branch veterinary clinics require an information system that integrates operational data while supporting information and technology governance. This study designs a web-based veterinary clinic information system for Cadika Veterinary Clinic using Scrum and COBIT 2019 through Design Factors, Enterprise Goals, Alignment Goals, Governance and Management Objectives, Capability Assessment, and Gap Analysis. The mapping identified nine objectives: EDM01, EDM03, APO02, APO13, BAI06, DSS01, DSS04, DSS05, and MEA01. Capability and gap values are simulation data rather than an actual organizational audit and were not derived from respondents. The target was set at level 4, while simulated current values were at levels 2–3; a gap of 2 was obtained for EDM03, APO13, DSS04, and DSS05, while the remaining objectives had a gap of 1. Black Box Testing covered 41 scenarios: 39 were documented as compliant, while two service-data scenarios had no recorded result in the source document. The system integrates clinic outlets, veterinarians, customers, services, reservations, examinations, and the COBIT 2019 module. Future work should validate capability with actual organizational evidence, conduct user acceptance testing, and measure operational impact.
